Yes, you can. You can add as many child tabs to existing tab as you want.

Can I modify existing profile tab sub navigation?

Yes, you can. You can reorder, change label, modify visibility and availability and do a few more things with it. You can not move existing sub tabs from one parent tab to another parent tab. You can link to existing sub tabs form other sub tab though.

Is it translatable?

Yes, it is!

Can I change the slug of tab or sub tab?

No. BuddyPress or BuddyBoss does not support changing tab slug yet( You may use custom code in PHP to define and change some of the top level slugs).
