I’m not sure if this is related, but we’re also experiencing an issue with BuddyPress.
When attempting to delete a term in wp-admin, we get a similar error message. We’ve discovered that the issue disappears when BuddyPress is deactivated.
This started occurring after we accumulated a large number of terms in a custom taxonomy called “Series”, which is used by the PublishPress Series plugin. At the moment, we have 440+ terms in that taxonomy, and the number is steadily increasing.
I haven’t yet tested reporting an item with Moderation Tools while BuddyPress is deactivated to see if there is a connection.

I am not sure but it seems there is something causing issue with database write. The reports are not being saved at all(Is there any chance the database crashed earlier? That might mark some tables read only).Regards

Were you able to see any error messages on your end?
I’ve run WP-Optimize and also performed a “check table” on all database tables, and everything appears to be OK. Is there any specific table I should inspect more closely?
Also, is it possible to test Moderation Tools with BuddyPress deactivated?
As I mentioned earlier, we’re seeing a strange issue when trying to delete a term in wp-admin — it results in an “unexpected error.” That problem disappears as soon as BuddyPress is deactivated.
Since BuddyPress behaves a bit oddly in that situation, I’m wondering if the two issues could be related.

Thank you for the reply.1. The Moderation tools currently depends on BuddyPress/BuddyBoss hooks and will ntot work without it.
2. The problem is with reports table

BrajeshYes, I’m able to post updates and comments in the activity stream, send messages, add and remove friends, etc., without any issues.
The problem only occurs in wp-admin when trying to create or delete a term (category, series, or tag) while BuddyPress is active — that’s when I get the error message. My assumption is that BuddyPress hooks into something after terms are created or deleted, and that the issue might be happening somewhere in that process.
As for front-end usage, I haven’t received any reports from users about problems creating content in BuddyPress.

2. is the reports table present and marked as clean?

Thank you for the reply.This is strange and I have never seen anything like this before. The problem is with report saving as the error message is from that action.
Do you have any staging environment? I will need to debug a bit(specially the insert query) and see what is causing the failure.
Please let em know if an staging environment is available and you can provide me temporary access to that.

Do you think this could be related to the other issue we’re experiencing with BuddyPress and terms, or are they likely unrelated?
At the moment, we don’t have a staging environment set up, but we’re planning to arrange one — it might take a bit of time, possibly until the weekend.
In the meantime, we could take the site offline for a maintenance window and create a full database dump right before you begin, in case anything goes wrong.

Thank you for the reply.I am not sure as I have never seen this issue. I will need access for 1-3 hours max(most probably less than 30 mins). I need access to sftp/ftp to allow me change file specially call to save method on Report) and trace the issue.
If you can allow me that widow, that would be perfect. My changes will have no affect on database but feel free to take a backup.
