Yes, it’s very strange.
Do you think this could be related to the other issue we’re experiencing with BuddyPress and terms, or are they likely unrelated?
At the moment, we don’t have a staging environment set up, but we’re planning to arrange one — it might take a bit of time, possibly until the weekend.
In the meantime, we could take the site offline for a maintenance window and create a full database dump right before you begin, in case anything goes wrong.
Let me know what you think.
I get the same error no matter what I try to report, user, post, BP activity, BBPress post or reply.
The table bpmts_moderation_reports exists and check table responds OK.

Yes, I’m able to post updates and comments in the activity stream, send messages, add and remove friends, etc., without any issues.
The problem only occurs in wp-admin when trying to create or delete a term (category, series, or tag) while BuddyPress is active — that’s when I get the error message. My assumption is that BuddyPress hooks into something after terms are created or deleted, and that the issue might be happening somewhere in that process.
As for front-end usage, I haven’t received any reports from users about problems creating content in BuddyPress.
