Hi Sheffie,
We tested it on a new digital ocean instance with php 7.3 and latest Maria DB and it worked. I am assuming the issue is our syntax might have problem in your version of Maria DB(we could not test with 5.5.60 as it was not available).
Can you please deactivate and then activate the plugin and check for database log. The error should give us a better pointer.

add a custom link in the menu and use http://yoursite.com/me
That will take logged in user to their profile.
You can also use htttp://yoursite.com/me/xyz to point to loggedinuser’sprofile/xyz

Thank you for sharing the details. The issue is most probably related to PHP/Mysql version if it is happening on all installs on the same server.
Please provide the mysql server version too.
Also, allow us to test it today on a similar configuration of PHP/Mysql and check why the table did not create on activation.
