Welcome to BuddyDev.I am sorry for the inconvenience but the problem lies in your current teme or template pack.
We do not include any template file. Instead we use
<?php bp_get_template_part( 'activity/entry' ); ?>which loads yourtheme/buddypress/activity/entry.php or wp-content/plugins/buddypress/bp-templates/{active-template-pack}/buddypress/activity/entry.php
Please check the responsible activity entry file. That should give some clue.
Also, if possible, Please post the code on pastebin and link me, I can check it for you.

Please make sure to enable the “Directory” for the specific member types. Then you can access the member types from http://yoursite.com/members/type/member-type-name/ (The plugin will list actual url on the member type list screen).Hope that helps.
